is a neuroaffirming, trauma-informed disability support provider based in Western Australia. We provide services designed to build capacity, promote independence, and improve quality of life for neurodivergent individuals and people with disability.Under the Privacy Act 1988 (Cth), Alyv is classified as a health service provider, and we are therefore bound by the Australian Privacy Principles (APPs). We collect personal information directly from the person we support, their nominee, guardian, or support coordinator during:

  • Intake and onboarding
  • Completion of service agreements and consent forms
  • Communications with stakeholders
  • Phone calls, emails, and text messages
  • Use of our Snap Forms and website
  • Interactions with our support workers and office team
  • Information shared via our app or in-person sessions

We collect personal information in order to:

  • Deliver safe, individualised, and effective disability supports
  • Understand and respond to a person’s goals, preferences, and needs
  • Coordinate with other stakeholders, such as plan managers and therapists
  • Maintain accurate service records
  • Fulfil our legal and regulatory obligations under the NDIS
  • Respond to feedback, complaints, or incidents
  • Improve our services and safeguard people’s wellbeing

5. How We Use and Disclose Personal Information
We use personal information only for the purpose it was collected, or a related purpose where the individual would reasonably expect it. This includes:

  • Communicating with the person we support and their nominated representatives
  • Providing support workers with the information they need to deliver safe and effective services
  • Managing bookings, scheduling, and documentation
  • Invoicing and billing via plan managers or self-managed arrangements
  • Meeting our reporting requirements to the NDIS Quality and Safeguards Commission or other lawful authorities
  • Respond to feedback, complaints, or incidents
  • Improve our services and safeguard people’s wellbeing

We do not share information with anyone else without consent, unless required or authorised by law (e.g., serious risk to health or safety).

You have the right to:

  • Access the personal information we hold about you
  • Request that we correct or update any inaccurate or incomplete information
